Description

Introduction

Xarelto 15mg is an oral anticoagulant that contains Rivaroxaban, which is a specific direct factor Xa inhibitor. Its purpose is to decrease the likelihood of developing thromboembolic complications through blocking a key component of the blood coagulation chain. Xarelto provides a simple, every-day or two-times-a-day regimen without the requirement for regular coagulation monitors, which makes it the preferred choice over conventional anticoagulants such as warfarin in the treatment of many patients.

Composition

  • Each tablet contains:
    Rivaroxaban – 15 mg
  • Inactive ingredients are microcrystalline cellulose, sodium croscarmellose, magnesium stearate, and various excipients to ensure tablet stability and bioavailability.

Pharmacological Class

  • Therapeutic Class: Anticoagulant
  • Pharmacological Class: Direct Factor Xa inhibitor
  • ATC Code: B01AF01

Mechanism of Action

Rivaroxaban is a selective and irreversible inhibitor of Factor Xa, a vital enzyme of the coagulation cascade that converts prothrombin into thrombin. Inhibiting factor Xa, Xarelto prevents thrombin production and fibrin clots, thus decreasing the chance of developing bleeding.

Indications

Xarelto 15mg is indicated for:

  • The prevention of strokes and embolism in patients suffering from non-valvular atrial fibrillation.
  • Treatment of deep vein thrombosis (DVT) and pulmonary embolism (PE)
  • Preventing chronic DVT and PE after the initial treatment
  • Prophylaxis of venous thromboembolism (VTE) in patients who undergo knee or hip replacement surgery.

Dosage and Administration

  • For treatment of DVT and PE:
15 mg once a day along with food during the initial 21 days. After that, 20 mg daily.
  • For prevention of stroke in atrial fibrillation:
20 mg a day when you eat.
  • For VTE prophylaxis after orthopedic surgery:
10 mg twice daily beginning 6-10 hours following surgery. This will last lasting between 10 and 14 days (knee replacement) or 35 days (hip replacement).
  • Tablets are best swallowed whole in water. They should be taken along in conjunction with meals to increase intake.
  • Adjustments to dosages may be necessary for renal impairment as well as certain patient groups; check prescription information.

Contraindications

  • Clinically significant and active bleeding
  • Hypersensitivity to rivaroxaban or any excipients
  • Hepatic impairment is severe due to coagulopathy
  • Prosthetic heart valves (use not established)
  • Pregnancy and lactation (use only if clearly needed)

Warnings and Precautions

  • A higher risk of bleeding. be aware of the signs of hemorrhage
  • Take care when treating patients suffering from impaired renal function; dosage adjustment may be required.
  • It is not recommended for patients suffering from mechanical heart valves
  • Do not use in conjunction with other anticoagulants unless it is clinically recommended
  • Consideration of elderly patients, as well as those who have an insufficient body mass or hepatic impairment
  • The risk of a spinal hematoma or epidural hematoma is higher who undergo neuraxial anesthesia, or puncture to the spinal canal

Drug Interactions

  • Use in conjunction with powerful CYP3A4 as well as P-glycoprotein inhibitors (e.g., ketoconazole or ritonavir) could increase plasma rivaroxaban levels, increasing bleeding risk
  • Inducers of CYP3A4 or P-gp (e.g. Rifampin, rifampin, or carbamazepine) can reduce their effectiveness
  • Take care when using antiplatelet agents such as NSAIDs, SSRIs, and any other medication that affects hemostasis because of the greater risk of bleeding

Adverse Effects

Common:

  • Bleeding events (epistaxis, gingival bleeding, bruising)
  • Gastrointestinal discomfort
  • Anemia

Serious:

  • Major bleeding (gastrointestinal, intracranial hemorrhage)
  • Hypersensitivity reactions (rare)

Patients are advised to notify any bleeding or bruising promptly.

Use in Special Populations

  • Renal impairment: Adjusting doses is recommended for patients suffering from moderate to severe kidney impairment (CrCl < 50 ml/min). The use of this medication is not recommended for patients who have a CrCl < 15 ml/min.
  • Hepatic impairment: Be cautious in patients who have significant bleeding from the liver, which can be caused by blood coagulopathy.
  • Pregnancy and lactation: Only use if the benefit is greater than the risk.
  • Elderly: Risk of bleeding increased; careful surveillance is recommended.

Monitoring

Regular coagulation monitoring (like INR) is not needed to use Xarelto, which gives the advantage over warfarin. However, periodic renal function and hemoglobin/hematocrit assessments are advised.

Overdose Management

  • There is no specific remedy readily available. Supportive care and the treatment of symptoms are essential.
  • It is recommended to use activated charcoal when the ingestion has been recent.
  • Prothrombin complex concentrates (PCCs) as well as recombinant factor VIIa might be considered for serious bleeding.

Storage

  • Store at room temperature (15–30°C)
  • Be sure to protect yourself from light and moisture
  • Make sure that the items are out of the reach of young children.

Patient Counseling

  • Eat with food for better absorption
  • Don’t skip dosages or stop them without consulting with an experienced healthcare professional
  • Inform health care providers about Xarelto treatment before surgery or a dental procedure
  • Be on the lookout for indications of bleeding, such as unusual bruising and blood in the stools. Also, look for signs of bleeding, such as nosebleeds, urine
  • Do not take any prescription or over-the-counter medication without consulting a physician

Conclusion

Xarelto 15mg (Rivaroxaban) provides a contemporary, efficient, and easy anticoagulant that has been proven effective in treating strokes and preventing the thromboembolic condition. The pharmacokinetics that are predictable, the oral administration, as well as the absence of standard monitoring requirements, make it an effective therapeutic option for clinical use.
