Description

Taffic 30mg is a fixed-dose antiretroviral medicine used in treating Human Immunodeficiency Virus-1 (HIV-1) disease. It is composed of three active ingredients:

  1. Bictegravir (30mg) – An inhibitor of integrase-strand transfer (INSTI) which blocks the transfer of DNA from viruses into your host’s genome.
  2. Emtricitabine (200mg) – a nucleoside reverse transcriptionase inhibitor (NRTI), which blocks the viral RNA-dependent DNA polymerase.
  3. Tenofovir alafenamide (25mg) – A different NRTI, which is a precursor drug of tenofovir. It is activated within cells and stops HIV replication.

A combination of these medications can reduce the burden of viruses inside the body, ensure immunity, and ward off progression to Acquired Immunodeficiency Syndrome (AIDS).

Indications:

Taffic is approved for treatment of HIV-1 in adolescents and adults (aged 12 and over who weigh less than 35kg) with the following characteristics:

  • Have no antiretroviral treatment history,
  • Are they virologically suppressed (HIV-1 RNA < 50 copies/mL) with a long-lasting antiretroviral treatment regimen for at most three months, with no evidence of treatment failure or known mutations that could cause resistance to the components of the regimen.

Dosage and Administration:

  • Recommended Dose: A tablet taken orally every day in combination with or without food.
  • Route: Oral
  • Form: Film-coated tablet

A dosage adjustment is not necessary for patients with mild or moderate kidney impairment. However, it’s not advised in patients suffering from extreme renal impairment (eGFR < 30 mL/min) or severely impaired liver function.

Mechanism of Action:

Each component functions at distinct phases of the HIV course of action:

  • Bictegravir: It blocks the HIV integrase enzyme. It prevents the integration of viral DNA into host DNA.
  • Emtricitabine and Tenofovir alafenamide: This block reverse transcriptase enzyme that is vital for the replication of viruses.

In combination, they decrease the capacity of HIV to reproduce and spread its virus into new cells.

Pharmacokinetics:

  • Bictegravir: Half-life ~17.3 hours
  • Emtricitabine: Half-life ~10 hours
  • Tenofovir alafenamide: Intracellular half-life >150 hours

The majority of the medication is transformed through the liver (CYP3A, UGT1A1), which is followed by the elimination of feces and urine.

Contraindications:

  • Hypersensitivity is a known reaction to a substance in the medication
  • Utilizing concurrently with medicines that have strong stimulants to CYP3A and UGT1A1 (e.g. Rifampin, rifampin, carbamazepine, St. John’s Wort) because they can diminish bictegravir’s levels and diminish the effectiveness of the drug.

Warnings and Precautions:

  1. Lactic Acidosis and Severe Hepatomegaly with Steatosis: In particular, with NRTIs. This includes deaths in some cases.
  2. Hepatic Flares in Co-infected Patients: Patients suffering from Hepatitis B (HBV) can experience acute flare-ups of the hepatitis following discontinuation of the treatment.
  3. Immune Reconstitution Syndrome: Inflammatory reactions can develop as the immune system is beginning to repair itself.
  4. Drug Interactions: Beware of rifampin, anticonvulsants (e.g., phenytoin), as well as herbal supplements that stimulate CYP enzymes.
  5. Renal and Hepatic Function: Check kidney function regularly, particularly for those suffering from kidney illness.
  6. Bone Density Effects: Small reductions in mineral density of bone have been seen in a few patients.

Side Effects:

Common Side Effects (≥5%):

Less Common but Serious:

  • Lactic acidosis
  • Hepatotoxicity
  • Hypersensitivity reactions
  • Exacerbation of HBV after discontinuation

Patients are advised to be aware of any symptoms that are unusual, including chronic fatigue, abdominal pain, or a yellowing appearance of the eyes or skin.

Use in Specific Populations:

  • Pregnancy: Limitation on data. It should be employed only in cases where the benefits outweigh the risks.
  • Lactation: Do not recommend. Breastfeeding mothers with HIV are not recommended as they are at risk of transmitting the virus.
  • Pediatrics: Accepted by patients aged ≥12 years and ≥35kg.
  • Geriatrics: Not much difference in terms of security. However, be cautious because of the possibility of decreased organ functioning.

Drug Interactions:

Avoid co-administration with:

  • Rifampin
  • Phenobarbital
  • Carbamazepine
  • St. John’s Wort
  • Antacids containing aluminum or magnesium (should be taken 2 hours apart)

Caution with:

  • Anticoagulants (e.g., dabigatran)
  • Certain statins (e.g., atorvastatin – may need dose adjustment)
  • Antifungals (e.g., ketoconazole – increases bictegravir levels)

Storage:

  • Store at room temperature (20°C to 25°C)
  • Place in the original container to keep moisture out
  • Be sure to keep it out of the reach of young children
