What is Tolvaptan?

Tolvaptan can be described as a selective vasopressin receptor V2 antagonist, which is employed to treat disorders that are related to water retention as well as lower blood sodium levels (hyponatremia). It aids the kidneys in the elimination of fluids without releasing electrolytes and thus fixing low sodium levels.

Hyponat O 15 mg refers to a fifteen mg tablet formula of Tolvaptan.

Mechanism of Action

Tolvaptan inhibits vasopressin (antidiuretic hormone) on V2 receptors inside kidney collecting ducts. Vasopressin is a hormone that normally increases the reabsorption of water; however, by shutting down this function, Tolvaptan blocks the following effects:

  • Enhances the excretion of free water (aquaresis)
  • The increase in sodium levels of the bloodstream is due to the reduction of dilutional hyponatremia
  • It does not result in any significant loss of electrolytes such as potassium or sodium

Indications

Tolvaptan is prescribed for:

  • Hyponatremia associated with:
    • Syndrome of inappropriate antidiuretic hormone secretion (SIADH)
    • Congestive heart failure (CHF)
    • Cirrhosis
  • Autosomal Dominant Polycystic Kidney Disease (ADPKD)
    • To stop the progression of the development of kidney cysts and the decline in kidney function.

Dosage and Administration

  • Starting dose for hyponatremia: 15 mg once a day could be increased to 30 mg, then 60 mg, based on your responses.
  • To treat ADPKD, Dosing begins with 60 mg a day (usually 30 mg once each day) and can be adjusted.
  • Taken early in the day to prevent the nocturia.
  • It is essential to monitor the blood sodium levels and the status of fluids throughout the treatment.

Side Effects

Common Side Effects:

  • Thirst and dry mouth
  • Increased urination
  • Polyuria (frequent urination)
  • Weakness or fatigue
  • Constipation

Serious Side Effects:

  • The overcorrection of sodium can cause osmotic demyelination syndrome (a severe neurological disorder) if serum sodium increases excessively.
  • Liver injury (particularly with prolonged use in ADPKD)
  • In the event of hypotension or dehydration, it is due to an excessive loss of fluid
  • The amount of water consumed is not enough

Precautions and Monitoring

  • The monitoring of sodium in the serum is crucial before starting treatment, at the time of initiation, and then regularly thereafter.
  • Do not attempt to correct sodium levels too quickly to reduce neurological issues.
  • Take care when treating patients with liver conditions. Tests for liver function are suggested.
  • Use caution in patients who aren’t able to feel or react to thirst.
  • It is not recommended for patients who are unable to take in enough fluids.
  • It is not recommended to have a baby or nurse unless it is clearly necessary.

Drug Interactions

  • Be aware of the use of other medications that can affect the levels of sodium.
  • CYP3A4 inhibitors (e.g., ketoconazole, clarithromycin, etc.) may boost Tolvaptan levels.
  • CYP3A4 inducers (e.g., rifampin) may reduce effectiveness.
