Description

Overview

Temonat 100mg is a cancer treatment that contains its active component Temozolomide. It’s part of a group of alkylating drugs that are used for the treatment of certain kinds of brain tumors. This includes glioblastoma multiforme (GBM) and anaplastic Astrocytoma.

Temozolomide is an oral agent for antineoplastic treatment that acts by disrupting the DNA of cancerous cells. This blocks their growth and results in the death of cells. It’s preferred because of its ability to traverse the blood-brain barrier successfully and is therefore suitable for tumors of the brain that can be difficult to treat.

Composition

Each capsule of Temonat contains:

  • Temozolomide — 100 mg

Inactive ingredients comprise starch gelatin, magnesium stearate, and other ingredients to guarantee that the capsule is stable and absorbs nutrients.

Mechanism of Action

Temozolomide is a drug that can undergo spontaneous hydrolysis when it is in a pH that is physiological into the active compound MTIC (monomethyl triazeno imidazole carboxyamide). MTIC alters DNA’s methylation on DNA at the O6 or N7 position on guanine. It damages DNA and induces Apoptosis (programmed end-of-life of cells) in tumor cells that are rapidly expanding tumor cells. It also inhibits the growth of tumors.

Indications

Temonat 100mg is indicated for:

  • Glioblastoma Multiforme (GBM):
    • Recently diagnosed GBM combined with radiotherapy as a treatment for maintenance.
    • Recurrent GBM after initial treatment.
  • Anaplastic Astrocytoma:
    • The treatment of patients suffering from anaplastic astrocytoma that is progressive or refractory.
  • Other High-Grade gliomas could be considered by Oncologists.

Dosage and Administration

  • Initial Dose for Newly Diagnosed GBM:
    • Concurrent with radiotherapy: 75 mg/m² per day for 42 consecutive days.
    • Maintenance therapy: 150-200 mg/m² every day for 5 days, every 28 days (28-day cycling).

  • Capsules are best swallowed whole in water. Ideally, it should be done on an empty stomach or at a minimum one hour before eating.
  • The need for dose adjustments is often necessary for cases of hematologic toxicity or any other adverse effects.
  • The length of the treatment is dependent on the patient’s responses and the degree of tolerance.

Contraindications

  • Hypersensitivity to Temozolomide and any ingredients that make up the composition.
  • Severe bone marrow suppression.
  • Pregnancy and lactation (due to potential fetal harm).

Warnings and Precautions

  • Myelosuppression: Temozolomide is known to cause extreme bone marrow deficiency, which can lead to neutropenia, thrombocytopenia, and anemia. Regular complete blood counts (CBC) are vital before and throughout treatment.
  • Infection Risk: Because of a lower immune system, patients are more at risk of contracting illnesses.
  • Secondary Malignancies: In the long run, prolonged usage can raise the chance of developing secondary cancers like myelodysplastic disorder or acute leukemia.
  • Hepatic and Renal Impairment: Be cautious in patients suffering from kidney or liver disease.
  • Pregnancy and Lactation: Contraindicated; A contraceptive that is effective should be employed.

Side Effects

Common side effects:

Serious side effects:

  • Severe myelosuppression
  • Opportunistic infections
  • Liver enzyme elevations
  • Allergic reactions (rare)

Patients must report any indications of an infection, unusual bruises bleeding, or fatigue immediately.

Drug Interactions

  • Other Myelosuppressive Agents: Take care with other medications that affect the bone marrow.
  • Antiemetics: Can be administered in conjunction with other medications to help manage nausea.
  • Live Vaccines: Avoid due to immune suppression.
