What is Sitagliptin?

Sitagliptin is an oral antihyperglycemic medication employed to treat the symptoms of type 2 diabetes mellitus. It is part of a group of medications known as DPP-4 inhibitors. These medications enhance blood sugar control by enhancing the body’s capacity to control glucose and insulin in response to food.

Mechanism of Action

Sitagliptin is a drug that works in the same way by blocking dipeptidyl Peptidase 4 (DPP-4), which reduces the incretin hormones GLP-1 (glucagon-like peptide-1) and GIP (glucose-dependent insulinotropic polypeptide).

  • Through stopping DPP-4, sitagliptin raises levels of incretin, and this:
    • The stimulation of the release of insulin by pancreatic beta cells occurs by releasing glucose in a glucose-dependent manner
    • Inhibit the release of glucagon in Alpha cells (which lowers hepatic sugar production)

The result is an improvement in glycemic control without leading to severe low blood sugar levels.

Indications

Istavel (Sitagliptin) is indicated for:

  • Diabetes mellitus type 2 in monotherapy (when both exercise and diet alone do not suffice)
  • Together with other anti-diabetic agents, like metformin or sulfonylureas, or insulin
  • Improve glycemic control and decrease HbA1c levels
  1. Dosage and Administration
  • Standard dose: 50 mg a day, in combination with or without food
  • In some situations or countries where the dosage may be different, it can be 100 mg twice each day, but always adhere to the local guidelines for prescribing.
  • Adjusting the dose may be required for patients with kidney impairment:
    • Mild renal impairment (eGFR ≥ 50 mL/min): no dose adjustment
    • Moderate renal impairment (eGFR 30-50 mL/min): 50 mg once daily
    • Severe renal impairment or ESRD (eGFR <30 mL/min): 25 mg once daily

  • Absorption: Absorption is rapid and nearly complete, with peak plasma concentrations 1-4 hours after the dose

  • Excretion: Primarily kidney-related (about 79% of the unchanged urine)

Side Effects

Generally well tolerated. Common side effects include:

  • Nasopharyngitis (common cold symptoms)
  • Headache
  • Upper respiratory tract infection

Less common/serious side effects:

  • Hypersensitivity reactions (angioedema, anaphylaxis, Stevens-Johnson syndrome – rare)
  • Pancreatitis (rare but serious)
